It sounds fraudulent on both of their parts but if he is a doctor he can sign your friend up for a year on state disability if your friend is too disabled to work. After that he has to file for permanent disability but that takes a long time to process.

If he was physically or mentally unable to work before ran out of unemployment benefits, then he must return the benefits received for the time when he was physically or mentally unable to work.
If he is physically and mentally able to work now, then he does not qualify for disability.
If he can prove that he happened to become physically or mentally unable to work at the same moment as he ran out of unemployment benefits, by coincidence, then this might work, maybe.

If he became disabled after he ran out of unemployment benefits then that would be o.k. Nothing fishy about that and what you stated is correct.
Only thing fishy is why he became disabled immediately when his unemployment ran out. If your disabled you are not eligible for unemployment.

When you’re unable to work, you no longer qualify for unemployment benefits.
If you become disabled at a time when you’re not employed, benefits don’t apply at all – after all, you weren’t working, so you’ve got NO income loss.
